Project Overview
The Dallas-Fort Worth Business Group on Health (DFWBGH) is
a coalition of 126 large DFW area employers and health care
stakeholders which fosters and supports regional health care
reform through advocacy, community outreach, education, collaboration,
consensus-building and consumer engagement.
In July 2007, DFWBGH launched a community-wide health care
quality improvement initiative, called the Partnership
for Peak Healthcare Performance (PPHP). The PPHP brings
together key stakeholders in an unprecedented collaboration
to improve the quality and outcomes of ambulatory care for
chronic illnesses which create a major health burden for the
community. Participating stakeholders have diverse perspectives,
but share a common interest: making sure all DFW residents
who suffer from chronic conditions receive the best care anywhere.
Together, we're trying to improve health care by making sure
employers, patients and doctors have access to the latest
and best information about evidence-based medical care, and
that care is accessed and provided in the best possible way.
The PPHP is doing that by providing information resources,
organizational support, knowledge and skills development,
team building, performance and outcome measurements, and more.
Why Diabetes?
About 1.3 million Texans are diagnosed with diabetes and
another 418,000 do not yet know they have it. Diabetes can
lead to serious and costly complications, and is the seventh
leading cause of death in the United States. The good news
is that diabetes can be controlled (and sometimes reversed!)
through proper treatment and making healthy lifestyle changes.
In fact, you can delay or even prevent getting diabetes by
knowing your risk factors and taking steps to combat them.
This Web site includes data and resources for physicians,
employers, and consumers. Together we can improve the health
of our community.
