Chronic diseases — such as heart disease, cancer, and diabetes
— are the leading causes of death and disability in the United
States. The good news is that chronic diseases are also among
the most preventable, and for certain conditions like diabetes,
a large body of research shows what actually works when it
comes to improving patients' health. The Partnership for Peak
Healthcare Performance (PPHP) promotes the use of such tools
and evidenced-based guidelines, and we've made it our mission
to measurably improve the quality of outpatient care for preventable
chronic illness in the Dallas-Fort Worth area.
Working with physicians, health plans, employers and consumers,
the PPHP will:
- Measure, improve and eventually publicly report the extent
to which physicians adhere to evidence-based treatment guidelines
for chronic illnesses.
- Educate and empower patients to proactively participate
in managing their own chronic conditions.
- Motivate consumers to demand better quality care for chronic
illnesses and to act on publicly reported information about
health care quality.
