Improving health care quality in DFW
Improving the quality of care in a community is the goal
and responsibility of all stakeholders, including physicians,
hospitals, employers, health plans, and patients themselves.
No individual, group or organization can accomplish this alone.
The Partnership for Peak Healthcare Performance (PPHP) brings
together stakeholders from across the DFW health care community
to work toward a shared goal of improving the quality of care
and outcomes related to chronic illness in the DFW Metroplex,
focusing first on diabetes care. With their expertise and
experience in directing patient care, physicians play a central
role in helping our community reach that goal. The PPHP offers
tools, resources and information to support physicians in
their quality improvement efforts.
For example, physician level Diabetes Care Reports
feature aggregated claims data across multiple payers, providing
physicians for the first time a broader view of the overall
level of care their population of diabetes patients is receiving,
compared to community-wide levels of treatment. Aggregated
summary reports of diabetes care management in the 12-county
DFW Metropolitan area features the same metrics used in the
physician level Diabetes Care Reports, compared to national
HEDIS quality measures. Click

The PPHP also offers physicians a unique opportunity to work
with other stakeholders to identify obstacles and explore
solutions for improving diabetes care. The Best Practices
section of this Web site provides opportunities for physician-to-physician
sharing of best practices to aid in clinical practice and
