Flow sheets for patient charts

Diabetes flow sheets remind you of the procedures and tests recommended for diabetes exams and simplify tracking of exam and test dates and results. Flow sheets facilitate documentation during office visits with diabetes patients, and make it simple for office staff to record test results and other information received in between visits, so you can quickly find this information on one page.

Printing flow sheets onto heavy, brightly colored paper makes them easy to find; add a tab and you or your staff can flip right to it.

Including an easy-to-identify diabetes flow sheet in the charts of each of your diabetes patients alerts you (or a covering physician) when a patient comes in that they have diabetes (regardless of the stated purpose of their visit) so you can better tailor treatment for their current condition. A quick review of the flow sheet will tell you whether the patient is up-to-date on tests and exams and allow you to address any concerns you have about their diabetes management while they are in the office.

Eye Exam Referral and Results Form

To facilitate communication between you and your patient's eye care professional, send your patient to his or her next eye exam with an Eye Exam Referral Form. The eye care professional can complete the form and fax it back to you to close the information loop.
