Optimal care bundle
Diabetes minimum standards
of care
Since 1995 the Texas
Diabetes Council has developed and continuously reviewed
and revised minimum standards of care to be followed when
treating patients who have diabetes. The standards are also
used to define minimum benefits for health plans regulated
by the Texas Department of Insurance. The Council's Medical
Professionals Advisory Subcommittee, a panel of medical experts,
develops the standards and treatment guidelines for the Council's
review and approval.
In recent years, some health care systems
have also adopted a "diabetes optimal care bundle"
to focus efforts on procedures which have the greatest impact
on reducing the risk of macro vascular disease and early death.
The diabetes optimal care bundle includes taking the
necessary steps to reach these targets:
 |
HbA1c < 7% |
 |
LDL cholesterol < 100
mg/dL |
 |
Diastolic blood pressure
< 80 mmHg, systolic blood pressure < 130 mmHg |
 |
Aspirin prescription for
patients age > 40 years |
 |
Non-smoking status |
Following are links to diabetes care algorithms
developed and endorsed by the Texas Diabetes Council, and
which address reaching the goals outlined in the diabetes
optimal care bundle.

standards of care protocols |
