Physician Forum
The Partnership for Peak Healthcare Performance (PPHP) was
created to improve care for chronic disease in DFW, starting
with diabetes care, by involving all stakeholders in the improvement
process. We believe the best source of diabetes care improvement
ideas is from DFW physicians themselves who are successfully
implementing these ideas in their practices.
The Physician Forum offers DFW physicians an opportunity
to share ideas and suggestions with their colleagues on what
they are doing to: meet the challenges of treating diabetes
patients, overcome obstacles in care management processes,
and improve overall delivery of care and patient outcomes.
We will update the Physician Forum quarterly with best practices
in diabetes care shared by local DFW physicians.
