Physician Forum

The Partnership for Peak Healthcare Performance (PPHP) was created to improve care for chronic disease in DFW, starting with diabetes care, by involving all stakeholders in the improvement process. We believe the best source of diabetes care improvement ideas is from DFW physicians themselves who are successfully implementing these ideas in their practices.

The Physician Forum offers DFW physicians an opportunity to share ideas and suggestions with their colleagues on what they are doing to: meet the challenges of treating diabetes patients, overcome obstacles in care management processes, and improve overall delivery of care and patient outcomes. We will update the Physician Forum quarterly with best practices in diabetes care shared by local DFW physicians.

Medical Clinic of North Texas piloting Patient-Centered Medical Home

In August, 2009 Medical Clinic of North Texas (MCNT) launched a Medical Home pilot with CIGNA Healthcare. Karen Kennedy, CAO of MCNT spoke at the November 17, 2009 Partnership for Peak Healthcare Performance Second Anniversary Dinner on the topic of "Medical Home and ACO's."

North Texas Doctors Offer On-Line Chat Version of Traditional House Call

The Dallas Business Journal reported that, "At least five North Texas doctors' groups have signed up to be among the first nationwide to offer patients a digital-age version of the traditional house call when they go live with online video chats." Learn how these physicians are using ".NowClinic, a new service designed to connect physicians and patients via virtual visits."
