DFW Diabetes Care Reports

Request missing or duplicate report

Measurement year 2008 Diabetes Care Reports are scheduled to mail in September, 2010. DFW Diabetes Care Reports are only produced for physicians who meet the following criteria in the measurement year:

Practicing in specialty of Endocrinology, Family Practice, General Practice or Internal Medicine.
Practicing in the 12-county DFW MSA.
Contracting with one or more of the following health plans: Aetna, Blue Cross Blue Shield of Texas, CIGNA, Humana or UnitedHealthcare.
Treated 20 or more eligible diabetes patients in the measurement year (see Methodology for specific patient eligibility requirements).

If you meet all these criteria and do not receive a report in September 2010 or if you need a duplicate copy of the DFW Diabetes Care Report for your practice, please complete the information below. Requests for CY 2008 reports must be received by October 31, 2010.
