Physicians - DFW Diabetes Care Reports

Request missing or duplicate report

The measurement year is the prior calendar year (for instance, data for calendar year 2008 is reported in December, 2009). DFW Diabetes Care Reports are only produced for physicians who meet the following criteria in the measurement year:

  • Practicing in specialty of Endocrinology, Family Practice, General Practice or Internal Medicine.
  • Practicing in the 12-county DFW MSA.
  • Contracting with one or more of the following health plans: Aetna, Blue Cross Blue Shield of Texas, CIGNA, Humana or UnitedHealthcare.
  • Treated 20 or more eligible diabetes patients in the measurement year (see Methodology for specific patient eligibility requirements).

If you meet all these criteria and did not receive a report in December showing prior calendar year data or if you need a duplicate copy of the DFW Diabetes Care Report for your practice, please complete the information below. Requests for reports which were mailed in December, must be received by January 15 the following year.
